There are two types of forms which are displayed. This A form type is defined chosen when setting up you forms are initially created (see creating forms):

• Validation forms (targeted response)
  Forms assigned to specific individuals, such as a field trip consent form sent to parents

• Registration forms (open access)
  Forms that can be submitted without explicit assignment, such as registration forms
