This form must be assigned through automatic generation by a Field Trip form, so it is essential to select a base form in the settings. The chosen base form must be a Field Trip form.
Field Tags
This form type allows for the use of System Tags to dynamically control your request forms. The most important tag will be:
Name: Helps identify each field trip request on the review page.
Base Form Map Source
This form is connected to its base form, enabling the use of the "SE - Base Form" map source. This feature allows you to import essential data from the base form, such as the field trip name, date, chosen activity, and more.

Important: When using the "SE - Base Form" map source, the only available option is "Matching Field Name". Ensure that the field names on both the Field Trip and Field Trip Confirmation forms match exactly to successfully pull in data. |
Accept or Decline
Unlike other forms in SchoolEngage, the submit button has been replaced with "Accept" and "Decline" buttons. This change allows parents or students to clearly indicate their decision to participate in the field trip, automatically updating the form's status based on their response.
